CRY myheart fund Agreement – ECG Testings and Research

Please complete and return if you want to ringfence your myheart Fund for ECG testing and Research.

During your association with us you will be privy to sensitive information. All Volunteers linked to CRY are required to behave in a responsible and ethical manner. CRY fundraising projects are the property of CRY. Information relating to such projects should not be distributed, photocopied, loaned or advertised to any other party without the written authorisation of the Chief Executive. Materials issued by CRY must not be used to promote any other cause or charity without prior permission from the Chief Executive. CRY will endeavour to support families in taking forward ECG testing. CRY Core Funding subsidises ECG Testing through funding research fellowship grants. If you should at any time change your mind and decide you do not want to take forward a ECG Testing Session in your local community you can request that CRY designate your ringfenced fund to CRY’s research fellowship grants. Please note that CRY reserves the right to use ringfenced funds to support the CRY screening and research programmes when a screening event is not organised within two years of funds being held within a ringfenced fund.
I agree to operate within the Guidelines as set out by CRY. I have read all the information attached to this agreement. I would like to ringfence the myheart Fund nominated above for Cardiac Screening and Research as of the date nominated against my signature (if the fund has already exceeded £3,500) or when the myheart fund reaches £3,500.
